Calories Burned:  692 calories per hour   (based on a body weight of 150 lbs.)
Instructions: 
You do this workout in multiples of 20 minutes
I keep the speed at 4.5 mph the whole time
From 0:00 to 5:00 minutes : Set the incline at 3.5
5:00 to 7:00 --> 10 incline
7:00 to 8:00 --> 6 incline
8:00 to 10:00 --> 10 incline
10:00 to 11:00 --> 7 incline
11:00 to 13:00 --> 12 incline
13:00 to 14:00 --> 10 incline
14:00 to 15:00 --> 12 incline
15:00 to 20:00 --> 4 incline (cool down)

(repeat if wanted)
I usually do 40 minutes of it with the cool down and warmup in the middle as a break.
